To join MOSTRA, please complete this form. Print the results and mail copy with your check in amount of $25.00 Payable to MISSOURI STATE TUMOR REGISTRARS’ ASSOCIATION (MoSTRA). Send to: Jeanie Shaneberger, CTR, MoSTRA Membership, 1705 East Broadway, Suite 240, Columbia, 65201 Membership is not active until dues have been received and processed. You will be notified when your membership has become active and given a sign-in and password.  After you have become active you may update your profile information by logging into the Members Corner.

 

Dues shall be paid on a yearly basis and shall be due on January 1 and considered delinquent on April 1, thereafter membership will be forfeited. A member will be considered in good standing and eligible to run for offices when the dues are paid by March 31. Dues paid between April 1 and August 31 will be applied to the current membership year. These members will be considered active members eligible to vote and or chair committees, but ineligible to run for offices. The MoSTRA membership year runs from January 1 through December 31. This membership is nontransferable.
